I solemnly swear that I was born at Sydney N.S.W. Australia on or about 22nd day of September 1874 that I ??? in the United States, sailing on board of the "Germanic" from Liverpool about September 1898, that I resided 15 years, uninterruptedly, in the United States, from 1897 to 1912 at San Francisco, California: that I was married to Edward(?) Charles Hetzel a native Aerican Oct 16 1901 - that he died in 1903 July in San Francisco: that I am the identical person described in said Certificate: that I am domiciled in the United States, my permanent residence being at Pittsburgh in the state of Pennsylvania, wher I have the occupation of ___: that I am about to go abroad temporarily, and intend to return to the United States within 5 years with the purpose of residing and performing the duties of citizenship therein: and that I desire a passport for use in visiting the countries hereinafter named for the following purpose:Australia to visit relatives, Great Britain to visit relatives.
